WSIB – INITIAL CLAIMS ASSISTANCE
- 1. Ensure you have reported the injury on a form 7 especially for Lost Time Claims. In the event that you only filled out an incident, get a copy of that from the employer. Submit that to the WSIB and request a Form 7. Keep a copy of your Form 7; if it has already been sent request a copy from WSIB. Most problems especially with wages occur as a result of the information on this form, and many employers submit a letter to the Board with it. The worker is entitled to this information too so request any additional information the employer has sent. If all else fails submit a Form 6 to start the claim.

- 2. Seek immediate medical treatment, either by your doctor, emergency room, chiropractor or a walk in clinic. That facility should have completed a Form 8, be sure to ask for a copy. Go back to the treating facility and request a copy if you do not already have one
- 3. Make sure that you told whoever treated you that it was a work related incident if you did not – fill out the attached medical waiver for the facility using the date etc and sign it. Then send this form to WSIB, this will save about 3 weeks on a claim if you do this and WSIB must have the original,also by sending in the waiver the Board will pay for the report instead of you. You, or yourr rep can access these later from the Board.
- 4. In the event of a gradual onset injury – practice with a blank piece of paper or a form 6 exactly what happened. When you receive the form 6 in the mail you will be able to correct any problems with the Form 7, by filling out the form 6 with more detail.For example – if you fell down the stairs – which way did you fall forward or backward? Were the stairs metal or wood? Did you roll or slide, if so what other body parts did you hit on your way down? How many stairs?
- 5. You should seek a qualified representative and sign an authorization over to them. This will allow them to speak to management on their behalf, the Board, or to access information. If you do not have a representative and you have more than one claim, especially if it involves the same area of injury, write down all claim numbers on the form and write a letter requesting those files. You may need them to prove a recurrence/aggravation injury as the Board usually gives the worker a new claim number as it is registered.
- 6. Remember if you are doing any type of gradual or modified work and are having problems to report it right away to the supervisor or your representative. Many workers work outside of their restrictions and put themselves at further risk of re-injury. If an injured worker works outside of their restrictions, the Board can cancel benefits. The worker is as responsible as the Employer.
- 7. If it looks like it is going to be a lengthy injury with lost time and has the potential to become problematic, remember your right to 15 weeks of sick leave benefits with Employment Insurance; all it takes is a doctors note. and have them apply early for CPP (should’nt this be a last resort?).
